Seven days on one island is the sweet spot. Five works if you stay put. Under five days the flying and the driving take too large a share of the trip.
Oʻahu. It has the widest range of things to do, the cheapest flights and accommodation and the only public transport worth using. It also suits groups who want different things from a vacation.
On every island except Oʻahu, yes, for the whole stay. On Oʻahu you can skip the car while you are based in Waikīkī and rent only for the days you leave town.
September and early October are usually cheapest, followed by late April and May. The most expensive stretch is December 20 to January 3, when rates often double.
